A Grave Mistake is a medium level 50 quest which focuses on The House of Twain's history. This quest was added in the Hallowynn update in 2016.

This quest requires the player to explore the graveyard and solve a series of puzzles.
Note: You cannot start this quest if you have not completed The House of Twain quest.

A strong spiritual force pulls you towards the graveyard, making it impossible to leave the area any other way than walking.
Dialogue:

To complete the quest, you'll need to visit places and get a piece of dialogue (listed below).
House across from the severed arm house. [278, 75, -472] Cathedral. [273, 82, -314] House with attic. [286, 83, -409] Severed arm house. [260, 75, -489] After visiting all places, you can go through the large door to advance to stage 3. |

- ???: But since you made it thusfar, I suppose I shall show you the truth...
- Mael: Do you understand now?
- Mael: Now leave, and forget the horrors you have witnessed here today...
- Mael: They are safer here.

The spiritual pull towards the graveyard loses its effect, making travelling easier.

Spoiler alert: The hidden text contains spoilers relating to the Severed Arm house.

You may notice that after entering, the door you entered through is a door out to the balcony, which cannot be jumped off. In order to get out, do the following:
Follow the bloodtrail upstairs to find the Severed Arm. Bring the Severed Arm to the hanged man.
A staircase will appear. Go up the stairs and look at the statue. |
{{HideSpoiler| In the House at [286, 83, -409] You will find keys in the first floor to open the door nest to the entrence. Also in the secound floor are keys which can be use in the third floor. Then you are in e room where a ghost appears.
Trivia
- This quest is the first one to use a new quest system which has been more used with the Corkus Update.
- Alem is an anagram of Mael
- Trick & Treat can no longer be obtained since Hallowynn 2016 is over
- There are many Easter-eggs hidden inside, such as a graves for the players JaydonTheWarrior and ethanoot.
